Initial evaluation

• At this visit, you and Dr. Haddad will discuss your intake questionnaire, past medical and psychiatric history, your personal background, and the concerns that have brought you into treatment. At the end of the session, Dr. Haddad will discuss her initial impressions and treatment recommendations, and you and Dr. Haddad will decide whether to continue working together. If you move forward with treatment, you and Dr. Haddad will discuss collaboratively an initial treatment plan.

• Please note that if you are pursuing couples therapy, each member of the couple will be required to meet with Dr. Haddad individually before starting treatment.
